my tax lady submitted the incorrect credit union and routing number for return deposit of my state and federal income tax......how can I provide the correct information to the IRS?

It is too late to make a correction once the return has been accepted or mailed.
It will be an inconvenience, but you shouldn't have anything to worry about. If the number given is invalid then the IRS will simply mail your refund in the form of a check. If the number is a valid account number for someone else's account, most banks will not deposit refunds when the names don't match up, and it would be returned to the IRS with a check issued to you. You can track your refund by calling the IRS at 800-829-1040 or using the "Where's my Refund" feature at www.irs.gov
