You should receive a statement from the non-profit agency to whom the donation was made. The statement should show the name of the recipient organization, the date donation made, date the acknowledgement was issued, the Fair Market Value and a listing of the items, and a statement to the effect that no goods or money were received by donor from recipient for their donation.
It is up to the donor to determine Fair Market Value.
