All Forms 8809, Application for Extension of Time to File Information Returns, filed on paper are now processed by the Internal Revenue Service Center in Ogden, Utah. These paper forms must be mailed. Faxes will not be accepted.

I think someone is using my name and information for wrong income statements.I did not work 2 jobs or have any medical for that job in 2006. I have already filed my tax return and received what I was owed on my federal. How do I fix this form? Thank you sincerely for your help.
